The remaining critical required fields appear after the membership requirement information. All of these fields except for the First Name must be the same as that provided on the membership application.

  1. First Name: If you have a Family Membership, the first name can be different from the first name associated with the membership but must have been one of the names listed in the Family Membership section of the membership application. If you have a Single Membership, the first name must be the same as the first name associated with the membership.
